Problem

- Aligned telehealth POS and modifier review with payer-dated rules for the dates of service in backlog.
- Separated credentialing/roster status from billing ownership so enrollment lag stopped being treated as a “coding problem.”
- Reported weekly: denial category mix, dollars touched, and repeat-denial frequency—not vanity overturn rates alone.
Operational outcomes observed
Within the first review cycle, the practice could see which denials were recoverable versus process-driven. Repeat authorization and telehealth POS failures declined after front-end checkpoints were adopted. Aging concentration shifted away from unmanaged 90+ day commercial balances that previously lacked a documented next action.
